The run makes it matter through the patient model, camera feed, room warning, enemy movement, or timer. Skip look up. Eye drops can calm it, but using them still puts players near the danger. The mistake that hurts the run is simple: Looking up drains sanity repeatedly, so curiosity can kill a run faster than leaving the room calmly.
