Sanity is the player health meter for staying alive through enemy hits, stress, fire, and ritual damage.
Sanity is the player health meter in Animal Hospital. Sanity acts as the player health meter. Each player starts at 61% sanity, while Intern starts at 71% because of bonus sanity. The run makes it matter through the patient model, camera feed, room warning, enemy movement, or timer. At low sanity the music slows; reaching 0% kills the player. Sanity loss comes from stress, enemies, fire, ritual candles, wrong taser use, and hostile events. The mistake that hurts the run is simple: Ignoring small sanity losses can leave a player dead before the next emergency starts. If the current server shows a different timer, shop text, room state, or animation, the live screen wins. The good read is catching Sanity early enough to save sanity, cash, items, or the patient on the bed.
Sanity is visible on the player UI during a shift. Gain it through coffee, chocolate, certain classes, healing/check-in perks, and some anomaly handling.

Psychologist is a 500 Animal Coins class that doubles sanity gains and sanity losses.
Nurse is a 20 Animal Coins class that starts the player with +1 inventory slot.
Sanity gets easier once the team calls the cue: the model, timer, shop card, room sign, CCTV feed, or diagnosis screen. That turns a scare into a decision instead of a panic click.
A clean read on Sanity turns the cue into the right action. Coffee goes to the Head Banger, Maple Syrup goes to Bed Monster, correct medicine follows the diagnosis screen, and Room 8 pressure gets surgery handling instead of random weapon fire.
A bad read treats Sanity like normal hospital traffic. That is how a suspicious patient gets admitted, a normal patient gets the wrong cure, a fire blocks a room, or a timer runs out while everyone is shopping.
